Things to know

Using WellNet Recommended Providers for imaging and non-emergency surgery can bring your cost to $0 — call WellNet before scheduling.

If you have access to group medical and prescription coverage through your spouse or domestic partner, the Spousal Advantage Plan reimburses your eligible out-of-pocket medical expenses — copays, coinsurance, and deductibles — at 100%, up to the Affordable Care Act out-of-pocket limits, with no premium deducted from your paycheck. A premium contribution reimbursement may also apply when your cost on the alternate plan exceeds what you would have paid here.

Enrollment requires waiving the New Motors medical plan and completing an attestation in your enrollment system. Certain alternate coverage doesn't qualify: Medicare, Tricare, Medicaid, exchange or individual policies, and HDHPs with active HSA contributions.

LifeTime Benefit Term with Long-Term Care — (855) 241-9891 · chubb.com

Life insurance that does double duty. It pays a cash benefit to your family if you pass away — money they can use for anything, like the mortgage, debt, or everyday bills.

You can also use it while you're living: if you ever need long-term care like home health care, assisted living, or nursing home care, the plan pays you 4% of your death benefit every month for up to 25 months, and your premiums are waived while you're receiving it. Extended care benefits can continue up to 50 additional months beyond that.

  • Your rate is locked in and will never increase
  • Keep your coverage if you change jobs or retire
  • Coverage available for your spouse, children, and dependent grandchildren
See full plan details in your Benefit Guide →
Things to know

Coverage is also available for your spouse, children, and dependent grandchildren. If you use your full death benefit on long-term care, the plan restores up to 50% of it (max $50,000) so your family still receives something.

Cash if it happens, and expert guidance the whole way. If you're diagnosed with cancer, this plan pays you $15,000 in cash — $5,000 at diagnosis and two $5,000 recovery payments over the following year — to spend however you need, on top of any other insurance you have.

It also includes genetics-based cancer support through healthŌme:

  • At-home genetic screening to understand your inherited cancer risk
  • A dedicated oncology nurse advocate
  • Clinical trial search and enrollment help, plus expert review of your treatment plan
  • Recurrence monitoring for up to three years after treatment

No medical questions to enroll, and it's HSA-compatible.
