Lincoln Financial Life & AD&D Employer-paid Basic Life & AD&D

1x salary for all benefit-eligible full-time employees

• Class 1: up to $300,000

• Class 2: up to $100,000

AD&D matches the life amount. Benefits reduce to 65% at age 65 and 50% at age 70, and terminate at retirement.

Voluntary Life & AD&D Includes Living Care/Accelerated Death Benefit, Waiver of Premium, Portability, and Conversion. Age reductions: 65% at 65, 50% at 70.

Lincoln Financial Disability

LTD own-occupation period: 24 months.

Coverage effective first of the month following the waiting period; late entrants require Evidence of Insurability.

MASA Access (Emergent Plus)

$14/month. $0 out-of-pocket for emergency ground and air ambulance, hospital-to-hospital transfer, and repatriation near home.

Coverage in the U.S. and Canada — no “out-of-network” ambulance.

Lincoln Financial Accident Insurance

Pays cash benefits for on- and off-the-job accidents, in addition to any other coverage.

• Hospital Admission: $1,250

• ICU Admission: $2,000

A lump-sum cash benefit paid directly to you after a covered diagnosis, usable for any expense.

• Employee & Spouse: $10,000 or $20,000 (spouse at 100% of employee amount)

Helps with medical and daily living expenses when you’re admitted or confined to the hospital.

Complete a Baseline Visit (virtual or in-person) within 120 days of your plan effective date and you’ll keep $0 deductibles and $0 copays for in-network care and preferred prescriptions all year.

Miss the window and the plan switches to a $5,000 deductible with copays and coinsurance.

The visit is free, confidential, and only one adult member needs to complete it to qualify a minor dependent.
