Cleanings twice a year. Endodontics, periodontics, and oral surgery are paid as basic; implants as major. No waiting periods.

• Frequency: exam 12 / lenses 12 / frames 24 months
• Lenses: single, bifocal, and trifocal covered in full after copay
• Frames: $150 allowance · Elective contacts: $150 allowance

Eligibility. You are eligible for benefits on the first of the month following 30 days from your date of hire, and must be actively at work to keep coverage. Dependent coverage is available to your legal spouse and children to age 26.  Dependent coverage is available to your legal spouse and children to age 26.

Changes outside Open Enrollment need a qualifying event. Marriage, divorce, birth or adoption, death, or a change in your spouse’s employment coverage — you have 30 days from the event to make a change. If you don’t enroll now, your next chance is the next Open Enrollment.

Premiums are pre-tax. Deductions come out under Section 125 before taxes, so elections lock for the plan year unless you have a qualifying event.

Using your coverage. Florida Blue ID cards are mailed after enrollment; a digital card is in the Florida Blue app. Find Value Choice providers and in-network doctors at floridablue.com. Find a dentist on the MetLife PDP Plus network and an eye doctor on the VSP Choice network at metlife.com.
