• Calendar year maximum: up to $1,000 per covered member · Deductible: $0 per visit
• Type 1 (exams, cleanings, X-rays, sealants): no waiting period
• Type 2 (fillings, root canals, protective restorations): no waiting period
• Type 3 (crowns, inlays, onlays, dentures): 12-month waiting period
• Child orthodontia: 50% to a $1,000 lifetime maximum, 12-month waiting period · not covered for adults

The plan pays a set maximum covered expense per procedure — see the schedule for amounts. You will not receive an ID card; your dentist verifies coverage by Social Security number.

Crum & Forster Accident Medical and AD&D — included with every MEC Plus plan

Cash benefits paid to you or your family to help with unplanned expenses after an accident. Underwritten by Crum & Forster and administered by NAHGA.

HealthiestYou by Teladoc — 24/7 doctors by phone or video, included with MEC Plus Select and Premier

U.S.-licensed physicians can diagnose, recommend treatment, and write non-controlled prescriptions. General medical visits are $0. Dermatology ($85), psychiatry ($220 initial / $100 follow-up), and mental health counseling ($90 per session) are available at negotiated rates. Register at healthiestyou.com.

Optum NurseLine — included with MEC Plus Value and Premier. Registered nurses answer health questions 24 hours a day.

CerpassRx — prescription benefits included with your MEC Plus plan

• Discount Rx Program (Select): pay the lesser of the pharmacy’s usual fee or the contract rate — savings of up to 85% on brand and generic drugs at more than 58,000 pharmacies, plus mail order for 90-day supplies

Florida Blue members use the pharmacy benefit built into the BlueOptions plan ($10 / $50 / $80, specialty $250).

Enroll or waive in Employee Navigator. Log in at employeenavigator.com; first-time users register with company identifier PriStaNig2026, the last four of your Social Security number, and your date of birth. Employer-paid coverage. Prime Staffing pays for employee-only MEC Plus Select and MEC Dental. You can buy up to Value or Premier, or elect the Florida Blue plan, and add dependents through payroll deduction.

Eligibility. You are eligible for benefits on the first of the month following 30 days from your date of hire, and must be actively at work to keep coverage. Changes outside Open Enrollment need a qualifying event. Marriage, divorce, birth or adoption, death, or a change in your spouse’s employment coverage — you have 30 days from the event to make a change. If you don’t enroll now, your next chance is the next Open Enrollment.

Premiums are pre-tax. Deductions come out under Section 125 before taxes, so elections lock for the plan year unless you have a qualifying event.

Using MEC Plus coverage. Preventive care must be at a PHCS network provider. Always ask whether a provider is in the PHCS Limited Benefit Network before booking, and show your American Worker ID card. Medical ID cards are mailed after enrollment; a temporary card is available at theamericanworker.com. You will not receive a card for dental — your dentist verifies Ameritas coverage by Social Security number.

Using Florida Blue coverage. If you elect the BlueOptions plan, your Florida Blue ID card is mailed after enrollment and a digital card is in the Florida Blue app. Find Value Choice providers and in-network doctors at floridablue.com.
