Plan year September 1, 2026 – August 31, 2027
Welcome to your Stratton Healthcare Center employee benefits site. Here you’ll find everything you need to understand and use your benefits for the 2026–2027 plan year — medical, dental, vision, and a lineup of voluntary coverage from Colonial Life.
Take a few minutes to explore each benefit, review what’s available to you and your family, and reach out to your benefits team with any questions along the way.
A few things people ask about most.
Open any coverage line for a clear, plain-language summary and the documents that go with it.
Florida Blue — three plans to choose from
BlueCare 71 ($4,400 deductible) and BlueCare 53 ($3,500 deductible) are in-network-only HMO plans on the BlueCare network. BlueOptions 5781 ($1,500 deductible) adds out-of-network coverage on the BlueOptions network. See the plan summaries below for copays, coinsurance, and out-of-pocket maximums.
Save with a Value Choice Provider. Primary care visits at Sanitas Medical Center or Diagnostic Clinic Medical Group locations are covered 100% on all three plans, with $20 specialist copays and onsite labs and X-rays. Find locations at mysanitas.com.
Download the Florida Blue app to view your ID card, check claims, find in-network doctors, and compare costs.
Dependent children covered up to age 26 (through the end of the year of their 26th birthday).
MetLife Dental — two PPO plans on the PDP Plus network
Out-of-network reimbursement at the 90th percentile UCR. Dependent children covered up to age 26.
MetLife Vision — Davis Vision by MetLife network
Dependent children covered up to age 26.
Colonial Life Insurance — voluntary, guaranteed issue
Permanent coverage that helps your family cover funeral costs, everyday bills, and more if the unexpected happens.
Colonial Life Disability Insurance — voluntary, guaranteed issue
If an accident, illness, or pregnancy keeps you out of work, disability insurance helps protect your income while you recover.
Some plans have a waiting period before covering conditions currently being treated — ask a Benefit Counselor for details.
Colonial Life Accident Insurance — voluntary, guaranteed issue
Helps cover costs for you, your spouse, or your children when the unexpected happens.
Colonial Life Critical Illness Insurance — voluntary, guaranteed issue
Provides cash to help you focus on recovery — not expenses — after a serious illness.
Colonial Life Hospital Indemnity — voluntary, guaranteed issue
Helps you handle medical and daily living expenses when you are admitted or confined to the hospital.
BlueVirtualCare — 24/7 virtual care, included with your Florida Blue plan
Video visits with board-certified doctors for common conditions, plus talk therapy (age 10+), psychiatry (18+), and dermatology photo review within 72 hours. Primary virtual visits are covered 100% on all three plans.
Log in to your Florida Blue member account or app, go to Find & Get Care → Find Virtual Care → BlueVirtualCare. For ongoing mental health support, Lucet can help you schedule an in-network provider at (866) 287-9569.
GoodRx is a free price-comparison tool — prescription prices can vary by more than $100 between nearby pharmacies. Compare at goodrx.com.
Amazon Pharmacy accepts most insurance plans and delivers to your door. Sign-up takes about 5 minutes.
Check local retailers like Publix, CVS, Target, and Walmart for no- or low-cost medication programs.
Everyone must complete enrollment — even if waiving. Speak with a Colonial Life Benefits Counselor at (844) 829-3674 (dial 1, 9am–5pm) or enroll online in Employee Navigator with company identifier STRATTONHCC. Enrollments are not submitted until you reach the Enrollment Complete page. New hires should enroll 30 days before their eligibility date.
Changes outside Open Enrollment need a qualifying event. Marriage, divorce, birth or adoption, a change in a child’s dependent status, a death in the family, or a change in employment or other coverage. You must notify Human Resources within 30 days of the event.
Your premiums are pre-tax. Health, dental, vision, and certain supplemental premiums are deducted before taxes under the Section 125 cafeteria plan — which is also why mid-year changes are restricted.
Who’s eligible: full-time employees working 30+ hours per week, plus your spouse or domestic partner and children up to age 26 (or older if unable to support themselves due to disability).
Pick the right place for care. A Value Choice Provider or virtual visit costs far less than urgent care or the ER. Save the ER for true emergencies, and use the cost estimator tools in your Florida Blue account before big-ticket tests or procedures.
Eligible: First of the month following 60 days from date of hire
