Your insurance coverage

WellNet Healthcare Medical — three plans on the PHCS/Open Network, each with an HRA

All three plans cover preventive care, primary care, and convenience care clinics at 100%, specialist visits for a $20 copay, and urgent care for a $50 copay. Virtual physician services through Sam by UCM (primary care, urgent and emergency care, mental health care) are covered 100%. Each plan includes an HRA benefit of $750 individual / $1,500 family that offsets your deductible and out-of-pocket maximum.

  • Bronze $5,000 Ded Plan: $5,000 / $10,000 deductible ($4,250 / $8,500 after HRA) · 20% coinsurance · $6,500 / $13,000 out-of-pocket maximum ($5,750 / $11,500 after HRA) · emergency room 20% after deductible · Rx $0 / $35 / $60 / 30% after deductible
  • Gold $2,500 Ded Plan: $2,500 / $5,000 deductible ($1,750 / $3,500 after HRA) · 10% coinsurance · $5,000 / $10,000 out-of-pocket maximum ($4,250 / $8,500 after HRA) · emergency room $500 copay · Rx $0 / $35 / $60 / 30% after deductible up to $500 max
  • Platinum $1,000 Ded Plan: $1,000 / $2,000 deductible ($250 / $500 after HRA) · 0% coinsurance · $4,000 / $8,000 out-of-pocket maximum ($3,250 / $6,500 after HRA) · emergency room $500 copay · Rx $0 / $35 / $60 / specialty $500

Independent clinical labs and free-standing X-ray facilities are covered 100%; hospital-based labs, imaging, and outpatient surgery cost more, so an office setting or independent center is the lower-cost choice. Register for the WellNet member portal and mobile app to view claims, download your ID card, and find a provider.

Dependent children are covered to age 26; coverage ends at the end of the calendar year.

Humana Dental — two PPO options on the PPO/Traditional Preferred network

  • Dental PPO High: $50 individual / $150 family calendar-year deductible (waived for preventive) · $3,000 annual maximum · preventive 100%, basic 90%, major 60% · orthodontia 50% for adults and children to age 19, $1,500 lifetime maximum

Three cleanings every 12 months on both plans. Endodontics, periodontics, and oral surgery are classified as basic services; implants are major. Out-of-network claims are reimbursed at the 90th percentile of usual and customary charges. Register at Humana.com to view your ID card, claims, and find a dentist.

Dependent children are covered to age 26; coverage ends at the end of the month of their birthday.
