Your insurance coverage

UnitedHealthcare — three plan options for team members outside Florida

Two Choice Plus PPO plans with out-of-network coverage and a Choice-network EPO that is in-network only. Preventive care is covered 100% in network on every plan, and Evo virtual visits are covered 100%.

  • UHC PPO HSA $5,000 (Choice Plus): $5,000 / $10,000 embedded deductible · 20% coinsurance after the deductible for office visits, urgent care, emergency room and hospital care · prescriptions $10 / $35 / $70 after the deductible · $6,850 / $13,700 out-of-pocket maximum · out of network covered at 50% after a $10,000 / $20,000 deductible · pairs with the PNC BeneFit Plus HSA
  • UHC EPO $5,000 (Choice): in-network only · $40 physician / $80 specialist / $85 urgent care / $350 emergency room copays · Designated Network lab covered 100% · 30% after the $5,000 / $10,000 deductible for X-ray, outpatient surgery and hospital care · prescriptions $10 / $35 / $70 · $7,600 / $15,200 out-of-pocket maximum
  • UHC PPO $2,000 (Choice Plus): $30 physician / $60 specialist / $85 urgent care / $350 emergency room copays · 20% after the $2,000 / $4,000 deductible for outpatient surgery and hospital care · prescriptions $10 / $35 / $70 · $6,850 / $13,700 out-of-pocket maximum · out of network covered at 50% after a $6,000 / $15,000 deductible

Advanced imaging (MRI, MRA, CT and PET) is covered at 50% after the deductible in network on all three plans; lab work costs least at a Designated Network lab. The voluntary GAP plan can reimburse the deductible and coinsurance costs marked with an asterisk in your guide.

Dependent children are covered to age 26; coverage ends at the end of the month of their birthday.

American Public Life MEDlink® GAP Plan — helps cover the deductible and coinsurance your medical plan leaves to you

A voluntary, limited-benefit plan that reimburses in-hospital and outpatient costs from your UnitedHealthcare plan. Always present your MEDlink ID card together with your medical ID card so your provider can file both claims.

  • Option 1: $1,000 in-hospital / $500 outpatient per covered person per calendar year (family maximums $3,000 / $1,500)
  • Option 2: $2,000 in-hospital / $1,000 outpatient per covered person per calendar year (family maximums $6,000 / $3,000)
  • Option 3: $3,000 in-hospital / $1,500 outpatient per covered person per calendar year (family maximums $9,000 / $4,500)

Outpatient benefits cover the hospital emergency room, urgent care, outpatient surgery, diagnostic testing and MRI facilities, physical therapy, ambulance, and outpatient treatment for a mental or emotional disorder. In-hospital benefits include confinement, ambulance, and inpatient mental health treatment (up to 30 days per covered person per calendar year).

MetLife Dental — Low PPO and High PPO on the MetLife PDP Plus network

  • Low PPO: $50 individual / $150 family calendar-year deductible (waived for preventive) · $1,000 annual maximum · preventive 100%, basic 50%, major 50% · no orthodontia
  • High PPO: $50 individual / $150 family calendar-year deductible (waived for preventive) · $2,500 annual maximum · preventive 100%, basic 90%, major 60% · orthodontia 50% for children to age 19, $1,500 lifetime maximum

Both plans pay the same percentages in and out of network, with out-of-network claims reimbursed at the 80th percentile of usual and customary charges. On the High PPO, non-surgical periodontics and simple oral surgery are covered as basic services; on the Low PPO they are covered as major. Endodontics, surgical periodontics, complex oral surgery and implants are major services on both plans.

Dependent children are covered to age 26; coverage ends at the end of the month of their birthday.

Superior Vision by MetLife — eye exams, lenses, frames and contacts

  • Exam: $10 copay · Materials: $10 copay
  • Frequency: exam 12 / lenses 12 / frames 12 months
  • Lenses: single, bifocal, trifocal and lenticular lenses covered in full after the copay
  • Frames: up to $200 after the copay · Elective contacts: up to $200

The VisionAccess discount program (program code MET2020) adds 20% off eye exams, lenses and lens options, and non-prescription sunglasses, 25% off frames, and discounts on laser vision correction.

Dependent children are covered to age 26; coverage ends at the end of the month of their birthday.

MetLife Life & AD&D — employer-paid basic coverage plus voluntary buy-up

Basic Life & AD&D (paid by Columbus Hospitality Management): 1x your annual salary, up to $500,000, for all active full-time team members. Benefits reduce 35% at age 65 and to 50% of the original amount at age 70.

Voluntary Life & AD&D:

  • Team member: $10,000 increments up to 5x your annual salary, maximum $500,000 · guarantee issue up to $250,000 at initial enrollment
  • Spouse: $5,000 increments up to 100% of your elected amount, maximum $250,000 · guarantee issue up to $50,000 at initial enrollment · you must elect coverage to enroll your spouse
  • Child(ren): $5,000 from 15 days to 1 year old and $10,000 over age 1, to age 26 for full-time students · you must elect coverage to enroll your children

AD&D pays the full principal sum for accidental loss of life or the loss of two or more limbs (hand, foot or eye), and half the principal sum for the loss of one.

Things to know

Guarantee issue ($250,000 team member / $50,000 spouse) applies at initial enrollment only.

The Standard Life Complete — permanent life insurance with a long-term care rider

Group whole life insurance that blends a traditional death benefit with long-term care protection and cash value. Your benefit can be used three ways: paid to your beneficiaries as a lump sum when you pass away; drawn as monthly payments to help pay for long-term care if you become chronically ill; or borrowed against or withdrawn from the accumulated cash value in a financial emergency.

Compared with stand-alone long-term care insurance, rates are typically fixed, qualifying is generally easier (a short questionnaire), and the benefit is not lost if you never need long-term care.

MetLife Voluntary Short-Term Disability — income protection for shorter absences

  • Benefit: 60% of your weekly salary, up to $1,000 per week
  • Elimination period: 7 days accident / 7 days sickness
  • Benefit duration: 12 weeks
  • Pre-existing condition period: 3 months / 12 months

Covers disabilities from a non-work-related injury or sickness. You must be regularly scheduled to work at least 30 hours per week and actively at work on the effective date; coverage begins the first of the month following your waiting period. Late entrants must provide satisfactory Evidence of Insurability.

MASA Access — Emergent Plus membership that covers emergency transport bills

MASA covers your out-of-pocket cost for emergency ground and air ambulance transportation to a medical facility, hospital-to-hospital transfers when specialized care is not available where you are, and repatriation to a hospital near home once your care provider approves the move. There is no out-of-network ambulance — send MASA the bill when it arrives and they work to get the charges covered.

Coverage territory: United States and Canada.

MetLife Accident Insurance — cash benefits for injuries on or off the job

  • Accident emergency treatment: $250–$300 · Follow-up visit: $125 · Therapy: $65 per visit
  • Hospitalization: $2,000 · Daily hospitalization: $500 · ICU: $1,000 per day · Ambulance: $400
  • Fractures, burns, concussion, coma: $250 to $12,000
  • Lodging: up to $300 per day · Accidental death: $225,000
  • Wellness benefit: $100 per person

Covers you and your family 24/7 — at work, at home, commuting, at school and playing sports.

MetLife Critical Illness Insurance — a lump-sum payment after a covered diagnosis

  • Benefit amount: $10,000 to $50,000 lump sum for heart attack, cardiac arrest, major organ transplant, kidney failure, stroke, coma, severe burns, paralysis, or loss of sight, speech or hearing
  • Cancer: 100% of the benefit amount · non-invasive cancer 25% · skin cancer 5% (not less than $250)
  • Coronary bypass: 50% of the benefit amount
  • Annual health screening benefit: $50 per person

MetLife Hospital Indemnity — cash to help with the costs of a hospital stay

  • First day of hospitalization: $1,000–$1,500 per calendar year
  • First day of ICU admission: $1,000–$1,500 per calendar year
  • Daily hospital confinement: $150–$300 per day, up to 31 days per calendar year

Nationwide Pet Insurance — My Pet Protection® plans through your workplace benefits

Choose My Pet Protection® or My Pet Protection® with Wellness500. Both plans are guaranteed issue, have a $250 annual deductible, and reimburse 50% or 70% of eligible costs for accidents, injuries, illnesses, hereditary and congenital conditions, diagnostics and imaging, and procedures and surgeries. Accounts, perks & good to know

PNC BeneFit Plus — Health Savings Account and Flexible Spending Accounts

Health Savings Account (HSA): pairs with the HSA medical plan. Contribute pre-tax through payroll (or on your own), pay for qualified medical expenses with the PNC BeneFit Plus debit card, and keep what you do not use — the balance rolls over every year, stays with you if you change jobs or retire, and can be invested in mutual funds once you meet the minimum balance. Annual contribution limits are set by the IRS, with a catch-up contribution if you are 55 or older.

Flexible Spending Accounts (FSA): set aside pre-tax dollars during enrollment for the year ahead. A Health FSA covers out-of-pocket medical, prescription, dental and vision costs; a Dependent Care FSA covers eligible daycare and dependent care expenses. Plan deductions and deposits are effective January 1, 2027.

Evo Virtual Care — $0 phone, video and Quick Care visits, 24/7/365

Everyday and urgent care (cold, flu, skin conditions, medication refills and more), weight management with evidence-based screening and medication management, talk therapy with licensed therapists, and mental health medication management for established prescriptions. Visits are $0, providers typically connect in under 15 minutes, and prescriptions are sent straight to your pharmacy.

To get care: sign up for a Patient Account on the Evo website using your work or personal email, set your password, and request care for yourself or your children. Adult dependents are invited from your Evo portal and create their own account.

MetLife + Aura Identity & Fraud Protection — integrated with the MetLife Legal Plan

  • Detection: monitoring and alerts for your credit report and score, personal information, social media accounts, financial accounts, and home and auto titles
  • Prevention: digital vault, privacy assistant, safe browsing, password manager, VPN and antivirus
  • Family protection: parental controls, child cyberbullying protection, child SSN monitoring and credit freeze wizard, and secure family sharing for up to 10 additional adults and unlimited minors
  • Resolution: 24/7 support, white-glove resolution service, lost wallet protection, and a $5 million insurance policy for each enrolled adult

MetLife Advantages — support services included with your MetLife life coverage

  • Grief counseling: face-to-face sessions with a licensed counselor, or by phone, for team members, dependents and beneficiaries coping with a loss or major life change
  • Funeral assistance: one-on-one help customizing funeral arrangements
  • Beneficiary claim assistance: expert guidance for beneficiaries working through their options and financial needs
  • Will preparation and estate resolution: unlimited consultations with an attorney, in person or by phone, including court representation, document preparation and associated tax filings when probating an estate
  • Travel assistance: emergency and non-emergency services when traveling 100 or more miles from home, including medical evacuation and repatriation, physician dispatch, prescription assistance, lost document and identity theft assistance, with a $1 million maximum per person per trip

How to enroll: elections are made in UKG during Open Enrollment or within your new-hire window. New hires are eligible the first of the month following 30 days of hire. Your elections stay in place for the full plan year unless you have a qualifying event.

Eligibility: full-time team members working 30 or more hours per week, plus your spouse or domestic partner and children to age 26 (and unmarried children over 26 who cannot support themselves because of a disability).

Qualifying events: marriage or divorce, birth or adoption of a child, a change in your child’s dependent status, death of a spouse or dependent, or a change in employment status or in coverage under another employer’s plan. Notify Human Resources within 30 days of the event; after 30 days, additions and deletions are denied until the next Open Enrollment.

Pre-tax premiums: medical, dental, vision and certain supplemental accident premiums are deducted before taxes under the Section 125 cafeteria plan.

Where to get care: start with an Evo virtual visit ($0) or your doctor’s office, and use urgent care rather than the emergency room when it is not a true emergency. Confirm that providers, labs and imaging centers are in network before scheduling — the in-network-only plans (HMO or EPO) have no out-of-network coverage.

Turning 65? Your prescription drug coverage is creditable for Medicare Part D, so you can delay Part D without a penalty while you are covered. See the Medicare page in your guide for how to get started.
