UnitedHealthcare — three plan options for Florida team members
Two Neighborhood Health Partnership (NHP) HMO plans on the NHP HMO/POS Access network (in-network only) and a UHC PPO on the Choice Plus network with out-of-network coverage. Preventive care is covered 100% in network on every plan, and Evo virtual visits are covered 100%.
Advanced imaging (MRI, MRA, CT and PET) is covered at 50% after the deductible in network on all three plans, and a Designated Diagnostic Provider (look for the green check in the provider search) gives you the highest level of benefit for imaging. The voluntary GAP plan can reimburse the deductible and coinsurance costs marked with an asterisk in your guide.
Dependent children are covered to age 26; coverage ends at the end of the month of their birthday.
American Public Life MEDlink® GAP Plan — helps cover the deductible and coinsurance your medical plan leaves to you
A voluntary, limited-benefit plan that reimburses in-hospital and outpatient costs from your UnitedHealthcare plan. Always present your MEDlink ID card together with your medical ID card so your provider can file both claims.
Outpatient benefits cover the hospital emergency room, urgent care, outpatient surgery, diagnostic testing and MRI facilities, physical therapy, ambulance, and outpatient treatment for a mental or emotional disorder. In-hospital benefits include confinement, ambulance, and inpatient mental health treatment (up to 30 days per covered person per calendar year).
MetLife Dental — Low PPO and High PPO on the MetLife PDP Plus network
Both plans pay the same percentages in and out of network, with out-of-network claims reimbursed at the 80th percentile of usual and customary charges. On the High PPO, non-surgical periodontics and simple oral surgery are covered as basic services; on the Low PPO they are covered as major. Endodontics, surgical periodontics, complex oral surgery and implants are major services on both plans.
Dependent children are covered to age 26; coverage ends at the end of the month of their birthday.
Superior Vision by MetLife — eye exams, lenses, frames and contacts
The VisionAccess discount program (program code MET2020) adds 20% off eye exams, lenses and lens options, and non-prescription sunglasses, 25% off frames, and discounts on laser vision correction.
Dependent children are covered to age 26; coverage ends at the end of the month of their birthday.
MetLife Life & AD&D — employer-paid basic coverage plus voluntary buy-up
Basic Life & AD&D (paid by Columbus Hospitality Management): 1x your annual salary, up to $500,000, for all active full-time team members. Benefits reduce 35% at age 65 and to 50% of the original amount at age 70.
Voluntary Life & AD&D:
AD&D pays the full principal sum for accidental loss of life or the loss of two or more limbs (hand, foot or eye), and half the principal sum for the loss of one.
Guarantee issue ($250,000 team member / $50,000 spouse) applies at initial enrollment only.
The Standard Life Complete — permanent life insurance with a long-term care rider
Group whole life insurance that blends a traditional death benefit with long-term care protection and cash value. Your benefit can be used three ways: paid to your beneficiaries as a lump sum when you pass away; drawn as monthly payments to help pay for long-term care if you become chronically ill; or borrowed against or withdrawn from the accumulated cash value in a financial emergency.
Compared with stand-alone long-term care insurance, rates are typically fixed, qualifying is generally easier (a short questionnaire), and the benefit is not lost if you never need long-term care.
MetLife Voluntary Short-Term Disability — income protection for shorter absences
Covers disabilities from a non-work-related injury or sickness. You must be regularly scheduled to work at least 30 hours per week and actively at work on the effective date; coverage begins the first of the month following your waiting period. Late entrants must provide satisfactory Evidence of Insurability.
MASA Access — Emergent Plus membership that covers emergency transport bills
MASA covers your out-of-pocket cost for emergency ground and air ambulance transportation to a medical facility, hospital-to-hospital transfers when specialized care is not available where you are, and repatriation to a hospital near home once your care provider approves the move. There is no out-of-network ambulance — send MASA the bill when it arrives and they work to get the charges covered.
Coverage territory: United States and Canada.
MetLife Accident Insurance — cash benefits for injuries on or off the job
Covers you and your family 24/7 — at work, at home, commuting, at school and playing sports.
MetLife Critical Illness Insurance — a lump-sum payment after a covered diagnosis
MetLife Hospital Indemnity — cash to help with the costs of a hospital stay
Amounts show the range across the plan options offered.
MetLife Legal Plans — attorney access for everyday legal matters
Telephone and office consultations, demand letters and document review on an unlimited number of personal legal matters. Choose from a network of more than 18,000 attorneys in all 50 states, use an attorney outside the network, or complete estate planning and download self-help documents online.
Covered matters include wills and estate planning documents, real estate and rental issues, reviewing leases, adoption, guardianship, name and gender marker changes, reviewing Medicare and Medicaid documents and parents’ estate plans, immigration assistance, student loan debt assistance, and more. Translation services are available for the call center and attorneys.
Nationwide Pet Insurance — My Pet Protection® plans through your workplace benefits
Choose My Pet Protection® or My Pet Protection® with Wellness500. Both plans are guaranteed issue, have a $250 annual deductible, and reimburse 50% or 70% of eligible costs for accidents, injuries, illnesses, hereditary and congenital conditions, diagnostics and imaging, and procedures and surgeries. The Wellness500 plan adds wellness exams, vaccinations, flea prevention, spay or neuter and more. Visit any vet anywhere, submit your claim, and get reimbursed. Nationwide also covers birds and exotic pets.
PNC BeneFit Plus — Health Savings Account and Flexible Spending Accounts
Health Savings Account (HSA): pairs with the HSA medical plan. Contribute pre-tax through payroll (or on your own), pay for qualified medical expenses with the PNC BeneFit Plus debit card, and keep what you do not use — the balance rolls over every year, stays with you if you change jobs or retire, and can be invested in mutual funds once you meet the minimum balance. Annual contribution limits are set by the IRS, with a catch-up contribution if you are 55 or older.
Flexible Spending Accounts (FSA): set aside pre-tax dollars during enrollment for the year ahead. A Health FSA covers out-of-pocket medical, prescription, dental and vision costs; a Dependent Care FSA covers eligible daycare and dependent care expenses. Plan deductions and deposits are effective January 1, 2027.
Evo Virtual Care — $0 phone, video and Quick Care visits, 24/7/365
Everyday and urgent care (cold, flu, skin conditions, medication refills and more), weight management with evidence-based screening and medication management, talk therapy with licensed therapists, and mental health medication management for established prescriptions. Visits are $0, providers typically connect in under 15 minutes, and prescriptions are sent straight to your pharmacy.
To get care: sign up for a Patient Account on the Evo website using your work or personal email, set your password, and request care for yourself or your children. Adult dependents are invited from your Evo portal and create their own account.
MetLife + Aura Identity & Fraud Protection — integrated with the MetLife Legal Plan
MetLife Advantages — support services included with your MetLife life coverage
How to enroll: elections are made in UKG during Open Enrollment or within your new-hire window. New hires are eligible the first of the month following 30 days of hire. Your elections stay in place for the full plan year unless you have a qualifying event.
Eligibility: full-time team members working 30 or more hours per week, plus your spouse or domestic partner and children to age 26 (and unmarried children over 26 who cannot support themselves because of a disability).
Qualifying events: marriage or divorce, birth or adoption of a child, a change in your child’s dependent status, death of a spouse or dependent, or a change in employment status or in coverage under another employer’s plan. Notify Human Resources within 30 days of the event; after 30 days, additions and deletions are denied until the next Open Enrollment.
Pre-tax premiums: medical, dental, vision and certain supplemental accident premiums are deducted before taxes under the Section 125 cafeteria plan.
Where to get care: start with an Evo virtual visit ($0) or your doctor’s office, and use urgent care rather than the emergency room when it is not a true emergency. Confirm that providers, labs and imaging centers are in network before scheduling — the in-network-only plans (HMO or EPO) have no out-of-network coverage.
Turning 65? Your prescription drug coverage is creditable for Medicare Part D, so you can delay Part D without a penalty while you are covered. See the Medicare page in your guide for how to get started.
Eligible: First of the month following 30 days
