  • PPO: the same in-network benefits as the EPO, plus out-of-network coverage at a $10,000 / $20,000 deductible, 50% coinsurance, and a $15,000 / $30,000 out-of-pocket maximum
  • PPO Max: the same in-network benefits, with out-of-network care at a $5,000 / $10,000 deductible, 20% coinsurance, and a $7,500 / $15,000 out-of-pocket maximum — and once your Baseline is completed, out-of-network services are covered at 100% of the usual and customary rate

Prescriptions (Baseline completed): $0 preferred (includes certain generic, brand-name, and specialty drugs), $50 brand and generic, $250 specialty. The EPO and PPO include Publix but exclude CVS and Walgreens; the PPO Max includes Publix, CVS, and Walgreens. Eligibility: full-time employees working 30 or more hours per week. New hires are covered the first of the month following 60 days of full-time employment. Eligible dependents are your spouse, children to age 26 (natural, step, adopted, or under your legal guardianship), and unmarried children over 26 who cannot support themselves due to a disability. The Curative medical plans cover dependent children to age 30.

Qualifying events: outside annual Open Enrollment, you can only change elections within 30 days of a qualifying event such as marriage, divorce, birth or adoption, a change in a child’s dependent status, the death of a dependent, or a change in employment status or in coverage under another employer’s plan. Notify Human Resources within 30 days — after that, additions and deletions are denied.

Pre-tax premiums: health, dental, vision, and certain supplemental accident premiums are deducted before taxes under the Section 125 cafeteria plan, which is why mid-year changes are limited to qualifying events.

Where to get care: complete your Curative Baseline Visit within 120 days of your effective date to keep $0 deductibles and copays, use Curative Telehealth at $0 any time, and remember preventive care is covered 100%. Consider a physician’s office, telemedicine, a convenience clinic, or urgent care before the emergency room, and compare prescription prices with GoodRx and retailer programs.
